Youth 1-to-1 Interaction Documentation Form

To maintain protection of all students and volunteers, please fill out this form for any one-on-one meetings or private interactions you have with any students outside of regular Sunday morning worship service or regularly scheduled youth group activities (see flowchart below). If you have any doubts about whether or not you should record an interaction, it never hurts to submit a record.

Written parental consent is REQUIRED for any activity with a minor. Indicate below how you attained parental consent.

Yearly registration covers events that are pre-scheduled by family ministry leadership. Interactions outside of those events need additional written parental consent which should be obtained by the youth leader prior to the activity. Please specify how you obtained this consent: GroupMe, text, email, etc.
